Only patients with a referral will be booked for an appointment.
If you are a primary care provider, please download the simple referral form below, fill it with needed information and fax to (905) 426 3306. Our referral can also be found on Ocean, Accuro, PS Suite, Epic and other EMRs.
Only a completed referral is needed. DBJS staff triages on a daily basis and will reach out to order any needed Ultrasound, Xray and/or MRI imaging.
